FORMES — Terms of Service (TOS)
Read Before Purchasing – Legally Binding Agreement
By booking an appointment, placing an order, making any payment, or completing checkout, you agree to these Terms of Service. In person appointments sign a consent form to agree to the following.
These Terms supersede all verbal communication or informal agreements.
1. Definitions

“FORMES” / “We” / “Us” / “Our” – FORMES Jewelry, its directors, staff, subcontractors, and subsidiaries. Including COLDFORMS Jewellery Ltd, FORMS and GRILLZ SUPPLY CO

“Customer” / “Client” / “You” – The individual purchasing or receiving a custom grill or jewelry item.
-
“Product” / “Piece” – Any grill, jewelry item, or service produced by FORMES.
-
“Deposit” – Any money paid prior to or during the moulding appointment.
-
“Mould” / “Impression” – A dental impression taken for the purpose of producing a custom grill.
-
"Design Orientation" – The final artistic, structural, and aesthetic choices made by FORMES during production.
-
"Fitting Appointment" – Any session where adjustments are evaluated or made.
2. Payments, Deposits & Financial Policy
2.1 Non-Refundable Deposit (Strict)
All payments made before, during, or after the moulding appointment—including booking fees, deposits, part-payments, and full payments—are strictly non-refundable.
Deposits immediately cover non-recoverable costs such as:

Precious metals and materials
-
Design work & CAD modelling
-
Laboratory work & mould creation
-
Artisan time and craftsmanship
-
Studio and operational expenses
-
Appointment slots reserved exclusively for the customer
By booking and/or paying, you agree these funds are used instantly and cannot be refunded under any circumstances.
2.2 Bespoke Work (Final Sale)
All FORMES products are custom-made using your unique mould and specifications.
Therefore:

All sales are final once a mould is taken
-
No refunds, cancellations, or exchanges are permitted
-
Custom work cannot be altered once confirmed
2.3 Cancellations & Rescheduling

Rescheduling is allowed with 24 hours’ notice
-
Any cancellation on missed appointments result in loss of all monies paid
-
No-shows are treated as cancellations
2.4 Final Balance
-
Your order must be paid in full before collection or shipment
-
FORMES may legally withhold the product until full payment is made
-
Unclaimed or unpaid orders may be disposed of after 60 days
2.5 Chargebacks and Payment Disputes
FORMES is committed to resolving any concerns directly with clients and always aims to ensure customer satisfaction. Clients are encouraged to contact us first so we can resolve any issues promptly.
If a client initiates a chargeback or payment dispute with their card provider without first attempting to resolve the matter with FORMES in line with the agreed contract, and the dispute is subsequently decided in favour of FORMES, an administration fee of £50 will apply.
This fee covers payment processor dispute charges and administrative time required to prepare and submit evidence.
The outstanding balance, including this administration fee, must be settled before the release or delivery of the piece.
3. Production Time Frames
3.1 Estimates (Not Guarantees)

Standard timeframes: 14– 42 days

Designs using CAD or premium materials: up to 12 weeks and beyond*
These times are guides only, not deadlines.
The customer acknowledges that:

Extended time frames do not qualify for refunds, cancellations, or disputes
-
FORMES may restart pieces for quality standards (and may not always notify the customer)
-
High order volumes or complexities may extend production
3.2 External Delays
FORMES accepts no liability for delays caused by:

Material shortages
-
Supplier delays
-
Quality control
-
Workshop issues
-
Acts of God or external events
Delays do not invalidate these Terms or qualify for refunds.
4. Design Orientation, Artistic Discretion & Aesthetic Variance
FORMES grills and jewelry are handmade, sculptural pieces produced through artisanal craftsmanship. Due to the bespoke and artistic nature of our work:
4.1 Artistic Interpretation
The customer acknowledges that FORMES retains full creative and structural discretion over:

Final shape and contouring
-
Surface finish
-
Stone placement
-
Symmetry relative to the mould
-
Design orientation and angle
-
Craft choices required for strength and safe wear
These decisions are made to produce the best final result based on:

The natural anatomy and alignment of your teeth
-
Structural limitations of the design
-
Metal thickness and durability requirements
-
Artistic judgement based on years of experience
4.2 Expected Variations
As a handcrafted piece, natural variations will occur. These include, but are not limited to:

Minor asymmetry
-
Slight variance in shine, polish, or texture or tone
-
Differences between the original design concept and the final form
-
Variations in edge thickness or contouring
-
Differences between digital previews and the final handcrafted product
These are normal, expected, and not grounds for refunds, remakes, or disputes.
4.3 Differences Between Customer Teeth & Final Design
Because every customer’s dental anatomy is unique, the final piece may appear:

More curved or more straight
-
Thicker in structural areas
-
Slightly different from inspiration images
-
Adjusted to fit safely and securely
FORMES may modify the design as required for:
-
Wearability
-
Comfort
-
Proper fitting
-
Structural integrity
-
Artistic finish
FORMES’ final design decisions are binding and non-negotiable.
4.4 Metal Colour, Alloy & Supplier Variation
Precious metals used in FORMES products are sourced from professional industry suppliers. Due to fluctuations in supply chains, availability, and alloy compositions:

FORMES may source metals from different suppliers or batches during production.
-
Gold alloys may vary in tone depending on their composition and refining process.
-
Lighting conditions, polishing methods, and finishing techniques may also influence the perceived colour of the metal.
As a result:
-
Differences in metal tone between pieces, batches, or previously produced grills may occur.
-
Digital previews, inspiration images, or previously seen pieces may not perfectly match the final metal colour.
These differences are normal within the jewellery industry and are not grounds for refunds, remakes, disputes, or complaints.
5. Fit, Adaptation & Completion Policy (Three-Phase)
FORMES uses a structured system to manage fit-related concerns.
5.1 Phase 1: Adhesive Fit Test (Required)
Customers must first attempt fitting the grill using Fixodent or Poli-Grip, as demonstrated.
If adhesive-fit is successful:

The piece is considered fully complete and accurately fitted
-
No remake or further adjustment is eligible
5.2 Phase 2: Grillz Fitting Appointment
If adhesive-fit does not resolve the fit concerns:
-
The customer may book a fitting/adaptation appointment
-
Minor structural adjustments may be made
Adaptations are limited to what is reasonable for the piece.
5.3 Phase 3: Final Assessment
If, after adhesive testing and adaptation:
-
The piece still does not fit the mould, and
-
There is no evidence of misuse, damage, or tooth movement,
Then a remake may be considered at the sole discretion of FORMES.
Important:
-
FORMES decides whether a remake is justified
-
FORMES decides whether a new mould is required (additional cost may apply)
-
FORMES decides whether the issue is customer-related
5.4 Timeline Reset for Adaptations/Remakes
If any of the following occur, the production timeline resets to Day 0:

Adaptation appointment
-
Customer-rescheduled appointments
-
Customer-requested changes
-
Remake (if approved)
-
Customer delay in responses
-
Missed or late fitting appointment
5.5 Completion of Order
Once the customer leaves with the piece:
-
The fitting agreement is complete
-
The order is fulfilled
-
Future fit changes are not eligible for adjustments, remakes, or refunds
- Any damage post-collection is the customer’s responsibility
5.6 Completion of Order Client Communication & Issue Notification Window
If the customer has any concerns, dissatisfaction, or issues relating to fit, comfort, or workmanship, these must be formally raised with FORMES within 14 days of collection or confirmed delivery.
Failure to notify FORMES of any issues within this 14-day period will result in the piece being deemed accepted and complete, and no further adaptations, remakes, or assessments will be considered.
Where FORMES has requested feedback, confirmation, attendance, or a response from the customer in relation to fitting, adaptations, or resolution of an issue, the customer must respond within a reasonable timeframe.
If communication from the customer ceases for a period of 7 consecutive days after reasonable attempts by FORMES to make contact:

The order will be deemed satisfactorily completed

Any open fitting, adaptation, or remake window will be considered closed
-
No further remakes, adjustments, or claims may be made
Fairness safeguard:
If the customer can demonstrate a genuine reason for non-response (such as medical emergency, travel, or circumstances beyond their control), FORMES may, at its sole discretion, allow the matter to be reviewed, provided contact is re-established promptly.
6. Reasonable Timeline Clause
A timeline of up to 6 months from the date of moulding is considered reasonable and fair for completion of bespoke jewellery and grillz work.
This period resets if:

Adaptations are requested
-
Customer changes or delays occur
-
A remake is approved
-
Fitting appointments are missed or rescheduled
This clause protects both parties.
7. Health, Safety & Wear Disclaimer

Grills are fashion accessories only
-
They are not dental devices
-
Do not wear while eating, sleeping, drinking, or exercising
-
Customer is responsible for oral health and safe wear
FORMES holds no liability for dental movement, discomfort, or injury caused by misuse.
8. Liability Waiver & Legal Jurisdiction
By purchasing, you agree that:

You use the product entirely at your own risk
-
FORMES is not liable for direct or indirect damages
-
All disputes fall under UK law
9. Acceptance of Terms
By paying a deposit, attending a moulding appointment, completing checkout, or collecting your piece, you confirm that:

You have read and understood the Terms of Service
-
You accept all sections without exception
-
You understand the bespoke, non-refundable nature of the product
-
You agree to the reasonable timeline and reset conditions
-
You accept artistic discretion and design variance
-
You accept your responsibilities for fitting, wear, and care
